The form serves as a preliminary form of application of the presentation.  Please send it preferably by the end of June 2009 to enable congress programme scheduling. If it is not sent by the 30th of September, it cannot be included into the "printed" congress programme already forwarded to the registered participants. According to the decision of the Scientific Committee it could be included into the congress programme.
